The safety and feasibility of hand-assisted laparoscopic nephrectomy (HALS) were evaluated in kidney transplant recipients. Ten patients were enrolled in a pilot study. A laparoscopic telescope was introduced through a stab incision in the right lower abdomen. Two right-handed and two left-handed surgeons participated. All renal transplant recipients were males with a mean age of 54 years. The mean duration of surgery was 110 minutes. The operative time for donors was 205 minutes and for recipients was 235 minutes. The recipient operation time was longer than that of donors. The mean donor graft size was 6.2 x 3.7 x 2.1 cm, and the mean recipient graft size was 3.7 x 2.7 x 2.1 cm. The donor and recipient sides were operated simultaneously. The complication rate was similar to that reported for laparoscopy in general. Hand-assisted laparoscopic nephrectomy is technically feasible and may provide a means of transplantation.
