High-dose methotrexate (HD-MTX) can be associated with severe neurotoxicity, with a reported incidence as high as 0.5%.\[[@ref1]\] Among patients receiving HD-MTX, 17-34% of patients develop signs of peripheral neuropathy.\[[@ref2][@ref3]\] Cutaneous involvement is infrequent, but in the literature, it is reported in 0.5-6.8% of patients, and can be manifested as persistent itchy, dry, vesicular, bullous, ulcerative, eczematous, erythematous or lichenoid eruptions.\[[@ref4][@ref5]\]\[[@ref6]\] In this report, we present a novel case of a 49-year-old female with colon adenocarcinoma and acute myeloid leukemia (AML) receiving HD-MTX therapy, who developed a severe, recurrent itchy, erythematous, rash affecting her lower extremities following the start of her HD-MTX.

CASE REPORT {#sec1-2}

A 49-year-old female with a past medical history of hypertension and myocardial infarction was referred to our tertiary dermatology service for further management of a 4-year history of a vesicular rash that had progressively worsened, mainly affecting the buttocks and extremities.
